• POSITION: Personal Care Aide

    REPORTS TO: Supervisor

    SCOPE OF RESPONSIBILITIES:

    Provides personal care service and assistance with activities of daily living to patients in their home.

    QUALIFICATIONS:

    • Personal Care Aide (PCA) Certificate from a Medicaid approved educational institution.
    • Successful completion of the Agency competency exam and skills checklist.
    • Personal Care Aide/Home Health Aide experience preferred.
    • Valid driver’s license and a reliable means of transportation to travel throughout the service area.
    • Ability to communicate effectively in both written and spoken English.
    • Ability to work in a cooperative and harmonious manner with patients, caregivers, coworkers, and HCC management.

    POSITION RESPONSIBILITIES:

    • 1. Performs personal care procedures and treatments under the direction of the supervisor.
      • A: Assists with care of teeth and mouth, grooming, care of hair, shaving, care of fingernails, and assisting with eating and dressing.
      • B: Assists patients with bathing or bathes patients.
      • C: Assists with moving, transferring, ambulating, and positioning patients as well as assisting with prescribed physical exercises as directed.
      • D: Takes temperature, pulse, respiration, and blood pressure as required and accurately records findings.
      • E: Prepares and serves meals for the patient.
      • F: Assists patients in the use of toilet facilities or changes patient diapers, as needed.
      • G: Assists with the administration of medications.
      • H: Measures and records fluid intake and output.
    • 2. Performs household duties that facilitate patient self-care in the home setting, such as light housekeeping chores related to the patient, washing the patient’s laundry, shopping, personal errands, and meal preparation for the patient.
    • 3. A personal care aide may not perform more than very limited general household work. General household work may not exceed 20% of the total weekly hours worked. General household work are tasks that do not directly benefit the client such as dusting, cleaning refrigerators or ovens, mopping floors, trash removal, or cleaning of household areas not used by the patient.
    • 4. Obtains information regarding services to be performed by reviewing the Plan of Care and follows the care plan accurately.
    • 5. Documents care provided on the Aide Record by filling out the necessary information and obtaining patient/caregiver signature along with the time slip.
    • 6. Records and promptly reports any changes in the patient’s condition, behavior, or appearance to the supervisor.
    • 7. Utilizes appropriate infection control measures and safety management procedures as appropriate.
    • 8. Demonstrates responsible management of resources.
      • A: Protects equipment and supplies from loss or theft.
      • B: Manages work time in an organized manner to accomplish tasks.
    • 9. Attends continuing education classes for a minimum of 12 hours per year.
    • 10. Keeps patient care information in strict confidence.
    • 11. Adheres to company work standards.
    • 12. Participates in organization-wide quality assurance activities as requested.
    • 13. Complies with company policies and procedures.

    WORKING ENVIRONMENT:

    Works indoors at the patient’s place of residence with travel from the home of one patient to another.

    PHYSICAL REQUIREMENTS:

    • 1. Ability to participate in physical activity.
    • 2. Ability to work for an extended period of time while standing and being involved in physical activity.
    • 3. Ability to do extensive bending, lifting, and standing on a regular basis.
    • 4. Ability to access home of assigned patient(s).
